Because of this, neurologists can act as consultants to other physicians or as your primary care doctor – depending how frequent your visits are required.

Rochelle Community Hospital teams with the Rockford Neuroscience Center to provide those dealing with these conditions with the care they need, right here in Rochelle. Mohammed S. Afzal, M.D. sees patients in Rochelle one Wednesday a month.

A physician referral is not required, but check with your insurance to determine if one is needed for you.

Your medical records will need to be forwarded to the office before your first appointment.

You will be greeted by our Multi-Specialty Clinic staff upon arrival to a neurology consult. Our staff will collect your photo ID and insurance information for registration. The Neurologist will then complete your consultation.
