Congratulations to Amy Hatfield, RCH Case Management Coordinator, for receiving our Third quarter Pillar of Excellence Award!

This quarterly award recognizes employees who display positive behaviors and characteristics as defined in our Dare to Soar standards, values and RCH mission. Employees receiving this award demonstrate these behaviors and characteristics on a consistent basis. Selection for this award recognizes these employees for their outstanding commitment and demonstration of concern, respect, and professionalism.

The Pillar of Excellence Award is a step above the typical ’employee of the month’ model of recognition. In order to receive this award, employees are nominated by a co-worker that fills out a nomination form that must fully illustrate how the employee demonstrates EXCELLENCE IN EACH CATEGORY of our Dare to Soar values. The application that meets the standards of the application as well as the award is chosen by our Employer of Choice team.

Commitment

“Amy provides resources for inpatients and outpatients. She is our “go-to” for any questions regarding care after discharge even if it means pulling some strings.”

Leadership

“Amy has offered improvement for her department by requesting a social worker position since we run into many cases where a social worker would benefit a patient’s situational outcome. She meets with many Home Health companies, Skilled Nursing facilities, and other reps to provide feedback of what more can be done on behalf of our hospital and patient care, and/or how much we appreciate the services the companies and facilities provide our patients after they leave our hospital.”

Professionalism

“Amy is always willing to go above and beyond to benefit the patients and the hospital. She comes out to the Med/Surg Nurse’s Station to communicate with staff as well as going to the Emergency Department to help with a patients discharge needs. She rounds with the physicians and answers any and all questions they may have and if she is unsure of the questions, she will try her hardest to find an answer. She represents Rochelle Community Hospital with high standard.”

Teamwork

“Amy is always asking for opinions of others on the care team (RN, CNA, HS, PT, OT, Dr, etc.) to determine the best outcome for a patient. She takes everyone’s time and thoughts into consideration and shows how much she appreciates everyone’s role. She picks up extra hours to cover her co-worker without hesitation and is willing to move her schedule around to help out in times of need so that the hospital isn’t left without a Case Manager during the day.”

Communication

“Amy not only rounds with the physicians, but also goes into each patient’s room to ask questions regarding their daily living and provides resources if needed. She provides the House Supervisor a report sheet when she won’t be here the next day or weekend to ensure the care plan doesn’t get lost in translation through shifts. She notifies the care team of any changes or concerns right away, and she meets with family members to discuss care of their loved ones.”

Ownership

“Many times, facilities may not accept a patient for various reasons. Amy never gives up and fights for the patient to get the best outcome after discharge. She notifies staff and family of these changes and provides them with what next steps are needed. She treats others like they’re her own family members, especially when patients don’t have anyone to advocate for them outside of the hospital setting.”

Innovation

“Amy is always adapting to patients with insurance changes and denials of placement or stay. She never gets discouraged and is willing to provide any information to continue the process of acceptance on behalf of the patient. She also helps provide resources for patients and/or their family members if they are not requiring placement and are unsure of what to do next. She tries her hardest to never let someone feel lost in their life and that there are always options.”

Excellence

“Amy is very considerate of other’s feelings during a time of need. She takes her time with patients and/or their family members to make them feel seen and heard. She helps patients all over the hospital (M/S, ED, FHC, etc.) and never makes anyone feel like their situation is less than someone else’s.

Amy is a substantial employee; she provides positivity to the workplace and is always there to help no matter what. She bends over backwards many times to ensure that a patient is being treated fairly and advocates for them to make sure their living situation is safe. Although sometimes the workload can be overwhelming, she never gets discouraged and continues to do her job the absolute best she can. I, along with any other coworker of Amy’s, can agree that she is key part of this hospital. She truly is a ray of sunshine on someone’s darkest day. If there was ever anyone to be a Case Manager for my family member, I would choose Amy.”
